What is Vestibular Neuritis?

Vestibular neuritis is a condition caused by inflammation of the vestibular nerve in the inner ear. This nerve transmits balance-related information to the brain. Inflammation disrupts this communication, resulting in severe vertigo.

Symptoms of Vestibular Neuritis

  • Sudden and severe vertigo (spinning sensation)
  • Imbalance
  • Nausea and vomiting
  • Worsening dizziness with head movements
  • No hearing loss (if hearing loss is present, the condition may be labyrinthitis)

Causes of Vestibular Neuritis

While the exact cause is unknown, it is often associated with:

  • Viral infections: Infections like the flu, common cold, or herpes virus are thought to affect the vestibular nerve.
  • Immune system issues: A weakened immune system can reactivate dormant viruses.

How is Vestibular Neuritis Diagnosed?

  1. Clinical evaluation: The doctor assesses the history of sudden vertigo and imbalance.
  2. Neurological examination: Testing eye movements (nystagmus) and balance.
  3. MRI or CT scan: To rule out other serious causes (e.g., stroke).
  4. Hearing test: Vestibular neuritis usually does not affect hearing; if hearing loss is detected, labyrinthitis is more likely.

Is Vestibular Neuritis Treatable?

Yes, vestibular neuritis typically resolves on its own. Treatment focuses on managing symptoms and accelerating recovery:

Medications:

  • Antihistamines or antiemetics to reduce nausea.
  • Vestibular suppressants to manage dizziness (recommended for short-term use only).
  • Antiviral drugs if caused by a viral infection.
  • Corticosteroids in severe cases.

Vestibular rehabilitation:

  • Exercise programs to promote faster recovery of balance.

 

How Long Does Recovery Take?

  • Acute symptoms usually subside within a few days.
  • Full recovery may take weeks to months, as the brain adapts to changes in the vestibular system.

Is Vestibular Neuritis Permanent?

Most people recover completely, but some may experience prolonged balance issues or mild dizziness. Vestibular rehabilitation is highly effective for managing persistent symptoms.

Conditions That Can Mimic Vestibular Neuritis

Vestibular neuritis can be confused with:

  • Labyrinthitis: Accompanied by hearing loss.
  • BPPV (Benign Paroxysmal Positional Vertigo): Linked to positional changes.
  • Meniere’s disease: Includes hearing loss, tinnitus, and vertigo episodes.
  • Stroke: Particularly in older individuals, as it can present with similar vertigo symptoms.

Can Vestibular Neuritis Be Prevented?

Direct prevention is not possible, but these steps may reduce the risk:

  • Practicing good hygiene to avoid viral infections.
  • Maintaining a healthy lifestyle to strengthen the immune system.

How to Manage Vestibular Neuritis?

  • Rest and avoid sudden movements.
  • Temporarily avoid head movements that worsen symptoms.
  • Perform vestibular rehabilitation exercises to improve balance.
  • Consult an ENT specialist or neurologist if symptoms persist long-term.
