What is Labyrinthitis?

Labyrinthitis is the inflammation of the labyrinth, the inner ear structure responsible for balance and hearing. This condition can cause vertigo, hearing loss, and other vestibular symptoms.

Symptoms of Labyrinthitis

  • Sudden and severe vertigo (spinning sensation)
  • Hearing loss (usually in one ear)
  • Tinnitus (ringing in the ear)
  • Imbalance or a swaying sensation
  • Nausea and vomiting
  • Fever or signs of infection (in some cases)

Causes of Labyrinthitis

Labyrinthitis may result from:

  • Viral infections: Viruses like flu, the common cold, or herpes.
  • Bacterial infections: Spread of a middle ear infection (otitis media) to the inner ear.
  • Autoimmune diseases: The immune system attacking the inner ear.
  • Trauma: Head injury or an injury affecting the inner ear.
  • Medications or toxic substances: Certain ototoxic drugs may rarely trigger labyrinthitis.

How is Labyrinthitis Diagnosed?

Labyrinthitis is diagnosed using:

  1. Clinical evaluation: Review of symptoms and medical history.
  2. Physical examination: Testing eye movements (nystagmus) and balance.
  3. Hearing test: Assessing the degree of hearing loss.
  4. MRI or CT scan: To rule out other serious causes like tumors or stroke.

How is Labyrinthitis Treated?

Treatment depends on the underlying cause and severity of symptoms:

Viral Labyrinthitis:

Usually resolves on its own. Supportive treatment may include:

  • Anti-vertigo medications (e.g., meclizine)
  • Anti-nausea medications
  • Rest

Bacterial Labyrinthitis:

A more serious condition often requiring:

  • Antibiotics
  • Possible hospitalization in severe cases

Vestibular Rehabilitation:

Recommended exercises to improve balance.

How Long Does Recovery Take?

  • Viral labyrinthitis: Typically resolves within a few weeks.
  • Bacterial labyrinthitis: Recovery may take longer and depends on the severity of the infection.
  • Balance issues may persist for months, but vestibular rehabilitation can speed up recovery.

Can Labyrinthitis Cause Permanent Hearing Loss?

  • Bacterial labyrinthitis or untreated cases may lead to permanent hearing loss.
  • Viral labyrinthitis usually allows full recovery of hearing.

Is Labyrinthitis Contagious?

Labyrinthitis itself is not contagious, as it results from an infection. However, the underlying infection (e.g., flu) may be contagious.

Difference Between Labyrinthitis and Vestibular Neuritis

  • Vestibular neuritis: Affects only the vestibular nerve, with no hearing loss.
  • Labyrinthitis: Affects both balance and hearing systems, causing hearing loss and tinnitus.

Can Labyrinthitis Be Prevented?

While direct prevention is difficult, these steps may reduce the risk:

  • Timely treatment of middle ear infections (otitis media).
  • Strengthening the immune system to prevent infections.
  • Vaccination against flu and other viral infections.

How Does Labyrinthitis Affect Quality of Life?

Severe vertigo and hearing loss can significantly impact daily life and work capacity. However, proper treatment and rehabilitation can greatly reduce these effects.
