What is an Acoustic Neuroma?

An acoustic neuroma is a slow-growing, typically benign tumor that develops from the Schwann cells surrounding the hearing and balance nerves (vestibulocochlear nerve). It is generally non-cancerous and does not metastasize.

Symptoms of Acoustic Neuroma

  • Unilateral hearing loss: Progressive, often affecting high frequencies first.
  • Tinnitus (ringing in the ear): Usually in one ear.
  • Dizziness or balance problems: Caused by involvement of the vestibular system.
  • Facial numbness or weakness: Due to compression of the facial nerve by the tumor.
  • Headaches and coordination issues: May occur with larger tumors.

Causes of Acoustic Neuroma

  • Genetic factors: While most cases are sporadic, neurofibromatosis type 2 (NF2), a genetic disorder, is a known risk factor.
  • Spontaneous mutations: The majority of acoustic neuroma cases are random and not linked to hereditary factors.

How is Acoustic Neuroma Diagnosed?

  • Hearing tests (audiogram): To determine the degree and type of hearing loss.
  • Magnetic resonance imaging (MRI): The most sensitive diagnostic method.
  • Computed tomography (CT): Used to evaluate bone structures.
  • Electronystagmography (ENG) or videonystagmography (VNG): To assess balance system function.

Can Acoustic Neuroma Be Treated?

Yes, treatment options depend on the tumor’s size, growth rate, and severity of symptoms:

  1. Observation: Regular MRI monitoring for small, asymptomatic tumors.
  2. Surgery: Removal of the tumor, with risks including hearing loss and facial nerve damage.
  3. Radiosurgery (Gamma Knife): A minimally invasive method to stop the growth of small or medium-sized tumors.

Growth Rate of Acoustic Neuroma

Acoustic neuromas generally grow slowly, but growth can sometimes accelerate. Regular follow-ups are essential to monitor the tumor’s progression.

What Happens if Acoustic Neuroma Is Untreated?

If left untreated, an acoustic neuroma can grow and cause:

  • Progression of hearing loss
  • Facial nerve compression leading to facial paralysis
  • Compression of the brainstem, potentially causing severe neurological complications (rarely life-threatening)

  • Hearing loss
  • Facial paralysis or asymmetry
  • Balance problems
  • Increased intracranial pressure (hydrocephalus) in large tumors

Who Is at Higher Risk for Acoustic Neuroma?

  • Individuals aged 30–60 years
  • People with neurofibromatosis type 2
  • Those with a family history of acoustic neuroma

Can Acoustic Neuroma Be Prevented?

There are no specific prevention methods for acoustic neuroma, but regular medical check-ups, especially for high-risk individuals, are important for early detection.

  • Hearing loss, tinnitus, and balance problems can significantly affect daily life.
  • Rehabilitation and supportive therapies after surgery or radiosurgery can enhance quality of life.

Recovery After Treatment

  • Post-surgery: Recovery of facial nerve function and balance may take weeks to months.
  • Post-radiosurgery: The tumor usually does not shrink but its growth stops, and symptoms may improve.
  • Rehabilitation: Balance exercises and physical therapy support recovery.
